Centricity Credit Union Medallion Signature Guarantee Requirements

Standard Requirements

  • Members requiring their signature to be guaranteed must be a member of Centricity Credit Union with an active transactional relationship for a minimum of 6 months
  • Accounts must be in good standing with Centricity Credit Union and have had no delinquencies, charged off accounts, or negative balances within the past 6 months
  • Medallion Signature Guarantee is not a same-day service and requires a pre-appointment interview to ensure all requirements will be met at time of service

Additional requirements

  • Proof of value – Centricity Credit Union is limited to guarantee signatures of $100K or less per transaction
  • A recent statement or current print of par value from the company website
  • Member(s) must have a valid (non-expired) Government issued ID to obtain a Signature guarantee
  • Proof that the Member(s) signing have the right to sign
  • Satisfy that the person signing is not being forced to sign
    • Statement- should have the members name on it unless it is for the deceased
    • Actual Stock Certificate
    • Death Certificates (court document showing appointment and authority)
    • Trust Agreements
    • Corporate Resolution
    • Power of Attorney
    • Additional documentation may be required
    • Stocks
    • Bonds
    • Mutual Funds
    • Savings Bonds
    • Unit Investment Trusts
    • Other Investment vehicles the definition of security
    • Savings Account Withdrawals
    • Bank Account Closeouts
    • Check Endorsements
    • Auto loan Applications
    • Certificates of Deposit
    • Amendments to Partnership Agreements
    • Amendments to Trust Agreements
    • Changes to Life Insurance Beneficiaries
    • Changes to Pension Trust Beneficiaries
    • Electronic Fund Transfer Enrollment Forms
    • Treasury Direct Administrative Account Updates
    • IRA’s that do not contain Securities
    • Copies of documents involved in a Security Transaction
    • Guaranteeing or certifying maintenance or administrative changes such as address, TIN updates, account number changes etc.
